About This Questionnaire
This questionnaire is not a medical diagnostic tool. It is designed to help you gauge your overall health and wellbeing, highlighting the areas where you may have a problem. Your results will help you to take the first steps to finding appropriate healthcare so that you can reach a balanced state of physical, mental and emotional wellness!
1. Overall wellbeing
- Do you feel happy?
- Do you feel healthy?
- Are you satisfied with everything about your body?
- If you answered NO to any of the above questions, elaborate (eg. my feet hurt all the time. I’m always tired)
The life of a modern woman is so busy, we hardly have time for self-evaluation and self- care. Take 30 seconds to STOP and do a wellness check on yourself by asking these simple questions. This is a good starting point on the road to better physical, spiritual and mental health.
2. Physical health
- Do you exercise LESS than 3 times a week?
- Do you suffer from regular pain anywhere in your body?
- Do you have a chronic illness?
- Do you think you are overweight?
- Is there something about your body that bothers you?

The human body is our very own well engineered machine. Sometimes there are parts that need servicing. We should not ignore the symptoms we feel. The minimum amount of exercise that an adult should be doing is 3x 30 minutes of any activity where you break a sweat, this is good for your heart, your muscles, and stress levels.

3. Nutrition
- Do you eat fast food more than 2 times per week?
- Do you eat fewer than five servings of fresh, raw vegetables and fruits per day?
- Do you sometimes feel bloated, full of gas, persistent fullness?

- Do you suffer from constipation or diarrhoea?
Have you heard the saying “you are what you eat’? this is so true!
The type of food you eat can affect your skin, your gut health, your risk of some cancers, your weight (of course!) your bowel movements, your mood, your acid reflux and indigestion issues.
Let’s actually throw away the word DIET and rather talk about a healthy eating plan because let’s be honest, we all crave not so healthy foods sometimes and this is fine occasionally and in small amounts.

4. Hormonal balance
- Do you constantly feel abnormally too HOT or too COLD?
- Do you feel constantly sad or depressed?
- Are you unable to lose weight despite everything you do?

- Is your sex drive abnormally low?
Hormones are one of those things that you don’t think about if all is well, but you will definitely notice when they are too high or too low! Thyroid hormone is responsible for regulating so many of the above-mentioned functions in your body.
Many women suffer from low levels of thyroid hormone and are completely unaware. The imbalance of thyroid hormone can be caused by many things. Some patients will require just medications to rectify the levels of thyroid hormone, while others need surgical removal of some or all of the thyroid gland.

5. Emotional and spiritual wellness
